Adults -- $125 per month

$100 enrollment fee for all new patients, applied towards first month

What is included in membership?

Unlimited visits and exams with your doctor
Telehealth, text and phone access
Home visits (location dependent)
Discounted blood work and diagnostics
Point of care testing (strep, flu, EKG, A1c, etc).

What will eventually be included in membership?

As I grow the practice, I hope to additionally offer common medications at cost to patients
